We request your copay at the time of service and will gladly assist you in filing insurance claims.  Please present your insurance information to our office staff at the time of your visit. Patients with HMO’s and PPO’s should check in advance to determine if we accept your plan.  If your insurance plan requires referral paperwork,  please have it available before, or at the time of, your visit.  Should your insurance carrier ultimately deny your claim and/or does not pay for the services rendered, you are still responsible for the bill incurred for the services performed. For your convenience, we accept Visa,  Mastercard, and Discover.
